Basically, Mafatu is afraid that the spearhead is sacred to the god of the cannibals and that he (Mafatu) will be punished if he takes it.  He fears that taking it from the altar of the god might cause his death.Mafatu had felt up to that time that Maui, god of fishermen, had led him to the island for a good reason.  But now, in front of the altar, he fears for the first time that he actually got to the island because of the god Moana, god of the sea.  He is afraid that this is Moana’s way of finally killing him.
