The reader first meets Holmes in this story in an opium den.  The den is located on Upper Swandam Lane in a very bad part of London.  It is described as something of a hellish place and Conan Doyle compares it to the forecastle of an emigrant ship.  This means that it is very cramped and unpleasant.  It is said to be reeking with smoke and one can see all kinds of miserable addicts in all sorts of poses.Holmes is there because he is looking for an enemy — he is there trying to solve a case.
